Heart Disease — 10-yr ASCVD (simplified)
For adults 40–79 without known ASCVD. This is a simplified estimator for demonstration and may differ from clinical calculators.
Diabetes — ADA Risk Test (lite)
A quick, score-based screener to flag who might benefit from A1c testing.
Stroke — FAST
Face drooping · Arm weakness · Speech difficulty · Time to call emergency services. If any are present, act now.

General
- • Blood pressure check: at least annually
- • Lipid panel: every 4–6 yrs (more often if high risk)
- • A1c or fasting glucose: if overweight/obese or other risk factors
- • Vaccines per schedule (flu, COVID, etc.)
Cancer screening (typical starting points)
- • Colorectal: usually start at 45 (colonoscopy or stool tests)
- • Breast: mammogram cadence per guidelines and risk discussion
- • Cervical: HPV testing/cotesting cadence
- • Prostate: discuss PSA screening around 50 (earlier if high risk)
